$125 per month for individual adults. There is a $100 enrollment fee; if the annual membership cost of $1500 is paid up front, the enrollment fee is waived.
$200 per month for an adult & spouse. There is a $100 enrollment fee; if the annual membership cost of $2400 is paid up front, the enrollment fee is waived.
$350 per month for a family of four. There is a $100 enrollment fee; if the annual membership cost of $4200 is paid up front, the enrollment fee is waived.
$400 per month for a family of 5 or more. There is a $100 enrollment fee; if the annual membership cost of $4800 is paid up front, the enrollment fee is waived.
